On-site Case Management and Social Services
Case Managers provide the case management service. Case Managers are Liaisons for the patients and the hospital’s care team. They communicate patient information to the referring hospitals. The Case Manager works with the physicians, staff, patient and families on discharge planning and psychosocial issues. The Case Managers ensure that the patient receives well-integrated, quality care. The Case Manager coordinates the interdisciplinary care of the patient. Social Workers are available to assist the patient with the best options during admission and upon discharge.
